ABSTRACT

Most biomedical journals now require authors to declare their conflicts of interest (COI), especially financial ones, before they accept the manuscript for submission. This study aims to examine the COI policies of Nepalese healthcare journals. The sample constituted journals indexed in Nepal Journals Online (NepJOL) as of June 2021. Of the 68 that met our inclusion criteria, 38(55.9%) journals endorsed the COI policy of the International Committee of Medical Journal Editors. Thirty-six (52.9%) journals had a policy for reporting the COI. Financial COI was the only type of COI mentioned. All journals in Nepal are encouraged to request the authors to declare the COI for better transparency.

ABSTRACT

In recent years, the issue of conflicts of interest in the transformation of scientific and technological achievements in medical institutions in China has become prominent, mainly manifested as personal and institutional conflicts of interest, with the characteristics of complexity and sustainability. At present, there were some problems in the conflict management of scientific and technological achievements transformation of medical institutions, such as insufficient support of relevant laws, regulations and policies, insufficient supervision of medical institutions, and the lack of industry management atmosphere. The author suggestted that government departments should strengthen the formulation of relevant policies and regulations, medical institutions should establish an interest conflict management system and an independent management department, and industry associations should give full play to their role in assisting, so as to provide reference for promoting medical institutions to effectively manage interest conflicts in the transformation of scientific and technological achievements.

ABSTRACT

Nos propusimos discernir en qué medida las estrategias mediáticas adoptadas en torno a las vacunas contra la COVID-19, a lo largo de los primeros 15 meses desde el comienzo de su aplicación, pueden considerarse aportes legítimos y coherentes para comprender mejor su desempeño, y en qué grado las narrativas elaboradas pudieran responder a intereses económicos de las élites corporativas. Una vez recopilados los elementos más relevantes con que se configuran las narrativas predominantes desde el momento en que se concibieron las vacunas, se identificaron diversas anomalías que resultaron, en mayor o menor medida, invisibilizadas en el proceso de su aprobación y de los resultados de su aplicación. Las más significativas conciernen al manejo de las definiciones, los incumplimientos de compromisos y los conflictos de interés que comprometen la actuación de las empresas comercializadoras y los entes reguladores de las vacunas. Numerosos elementos relacionados con los intereses corporativos han gravitado en la elaboración del relato sobre las vacunas. Entre los que reclaman resignificación se hallan: su capacidad preventiva real de contagios, evoluciones graves y muertes, su eficacia ante nuevas variantes, la duración de la inmunidad que confieren, sus efectos adversos, el papel sinérgico de la inmunidad adquirida y los recursos empleados por las empresas para conseguir un predominio virtualmente monopólico en el mercado.


We set out to discern to what extent the media strategies adopted around the vaccines against covid-19 during the first 15 months since the beginning of their application can be considered as a legitimate and coherent contributions to a better understanding about their performance and to what extent the elaborated discourses could respond to the economic interests of the corporations' elites. Once the most relevant elements that have shaped the predominant narratives from the moment the vaccines were conceived were compiled, several anomalies were identified that were to a greater or lesser extent invisibilized in the process of their approval and the results of their application. The most significant of these concern the handling of definitions, non-compliance with commitments and conflicts of interest that compromise the actions of vaccine marketing companies and regulatory bodies. Many elements related to corporative interests have influenced the narrative concerning vaccines. Among those that require a resignification are: their real preventive capacity against contagions, serious diseases and deaths, their efficacy against new variants, the duration of the immunity they confer, their side effects, the synergic role of acquired immunity, and the resources used by the companies to achieve a virtually market monopolistic predominance.


Propomo-nos discernir em que medida as estratégias mediáticas adotadas em torno das vacinas covid-19, ao longo dos primeiros 15 meses desde o início da sua aplicação, podem dar contribuições legítimas e coerentes para melhor compreender o seu desempenho, e em que medida as narrativas poderiam responder aos interesses econômicos das elites corporativas. Uma vez compilados os elementos mais relevantes com os quais se configuram as narrativas predominantes desde o momento em que as vacinas foram concebidas, foram identificadas diversas anomalias que foram, em maior ou menor medida, invisibilizadas no processo de sua aprovação e nos resultados de sua aplicação . As mais significativas dizem respeito à gestão de definições, descumprimentos de compromissos e conflitos de interesse que comprometem a atuação das empresas comercializadoras e dos órgãos reguladores das vacinas. Inúmeros elementos relacionados a interesses corporativos gravitaram na elaboração da história sobre vacinas. Entre os que demandam ressignificação estão: sua real capacidade preventiva de contágios, evoluções graves e óbitos, sua eficácia contra novas variantes, a duração da imunidade que se consolidou, seus efeitos adversos, o papel sinérgico da imunidade adquirida e os recursos utilizados. empresas para alcançar o domínio do monopólio virtual no mercado.

Resumo Este artigo busca investigar conflitos de interesses envolvendo a apresentação de ensaios clínicos em congressos brasileiros de cinco especialidades médicas, ocorridos entre 2004 e 2018. Foram estudados 407 resumos em 22 anais. Após aplicar critérios de seleção, obteve-se um corpus de 77 ensaios. Detectou-se maior frequência de conflitos de interesses envolvendo ensaios com drogas para as quais não havia genéricos/similares ( p =0,000), sendo que em 48% daqueles em conflito de interesses não houve declaração. Os resultados favoráveis à droga-teste ocorreram em 90,9% do total de ensaios, mas em 48,6% deles não foi reportado valor de p . As categorias terapêuticas mais testadas foram imunossupressores e imunomoduladores, antidiabéticos e antineoplásicos, que, juntas, representaram 68,9% do total de drogas envolvidas. Os resultados apontam conflitos de interesses ocultos, supervalorização de resultados positivos de drogas-testes, nem sempre com evidências suficientes, e concentração de produção em drogas de alto custo.


Abstract This article seeks to investigate conflicts of interest involving the presentation of clinical trials in Brazilian congresses of five medical specialties between 2004 and 2018. A total of 407 abstracts in 22 annals were studied. After applying selection criteria, we reached a corpus of 77 essays. A higher frequency of conflicts of interest was found involving essays with drugs for which no generic/similar option was available (p=0.000), and 48% of those with a conflict of interest declared nothing. Favorable results to the test drug occurred in 90.9% of the total of essays, but 48.6% of them lacked the p-value. The most tested therapeutic categories were immunosuppressors and immunomodulators, antidiabetic, and antineoplastic, which, together, amounted to 68.9% of the total of the involved drugs. The results pointed to hidden conflicts of interest, overvaluing of positive results of test drugs, not always with sufficient evidence, and focus of production on high-cost drugs.


Resumen Este artículo analiza los conflictos de intereses en ensayos clínicos presentados en congresos brasileños de cinco especialidades médicas, realizados entre 2004 y 2018. Se analizaron 407 resúmenes de 22 anales. Tras aplicados los criterios de selección se obtuvo un corpus de 77 ensayos. Hubo una mayor frecuencia de conflictos de intereses en ensayos con medicamentos para los que no había medicaciones genéricas/similares ( p =0,000), y el 48% con conflictos no hubo su declaración. Los resultados favorables para droga prueba están en el 90,9% del total de ensayos, pero el 48,6% de ellos no informó el valor de p . Las categorías terapéuticas más probadas fueron inmunosupresores e inmunomoduladores, antidiabéticos y antineoplásicos, que juntas compusieron el 68,9% del total de fármacos. Los resultados apuntan a conflictos de intereses ocultos, sobreestimación de los resultados positivos de las drogas prueba, no siempre con evidencia suficiente, y concentración de la producción en medicamentos de alto costo.

Abstract: In recent decades, the growing obesity epidemic in Mexico has become one of the most important public health challenges faced by the country. With support from the World Obesity Federation, we formed a working group in 2021 to identify and summarize priority actions that Mexico can take to face this epidemic. More than 1 000 health professionals joined the development and discussion process. Recommendations from previously published, high-level documents and guidelines were taken into account. In commemoration of World Obesity Day 2022, this statement is presented as input for health care professionals to develop actions to address obesity. The statement includes 10 recommendations that include population-level and individual-level actions. It emphasizes the importance of social participation, comprehensive interventions with a person- centered perspective, planetary sustainability, on improving education and communication campaigns, as well as fostering a built environment that promotes active living, and shielding prevention and control efforts from conflicts of interest. The statement calls for obesity to be treated seriously, based on scientific evidence, in a timely and comprehensive manner, employing a life-course and ethical approach that does not perpetuate weight stigma in society.

Resumo O objetivo do estudo é analisar problemas morais e/ou éticos vivenciados por membros de comitês de ética em pesquisa durante suas atividades e as estratégias utilizadas para solucionar esses problemas. Trata-se de pesquisa de abordagem qualitativa, com participação de 39 membros que atuavam em comitês em Salvador/BA, Brasil. Os dados foram coletados em outubro de 2020 por questionário on-line autoaplicado na plataforma Google Forms. Os resultados evidenciaram: vivências de conflitos de interesses e de valores, bem como dilemas; e utilização de trocas de experiências e compartilhamento de ideias e opiniões como estratégias para solucionar esses problemas. Conclui-se que os membros desses comitês vivenciam problemas morais e/ou éticos e buscam solucioná-los por meio de estratégias que favorecem o desenvolvimento de pesquisas conforme critérios éticos e metodológicos adequados.


Abstract The objective of the study is to analyze moral and/or ethical issues experienced by members of research ethics committees when performing their activities and the strategies used to deal with those issues. This is a qualitative study with 39 individuals that participated in committees in Salvador, Bahia, Brazil. Data were collected in October 2020 using a self-administered online questionnaire developed using Google Forms. Results showed that participants underwent conflicts of interests and values, and also faced dilemmas, and the strategies used to deal with those issues were exchanging of experiences and sharing of ideas and opinions. It is concluded that members of such committees experience moral and/or ethical issues and seek to deal with them via strategies that favor the development of research according to appropriate ethical and methodological criteria.


Resumen El objetivo del estudio es analizar los problemas morales o éticos experimentados por los miembros de los comités de ética de investigación durante sus actividades y las estrategias que utilizan para resolver dichos problemas. Se trata de una investigación de enfoque cualitativo, con la participación de 39 miembros que actuaban en comités en Salvador, Bahía, Brasil. Los datos se recopilaron en octubre del 2020 por medio de un cuestionario en línea autoadministrado en la plataforma Google Forms. Los resultados mostraron experiencias de conflictos de intereses y valores, así como dilemas; y el intercambio de experiencias, ideas y opiniones como estrategias para solucionar dichos problemas. Se concluye que los miembros de estos comités experimentan problemas morales o éticos y tratan de solucionarlos mediante estrategias que favorezcan el desarrollo de investigaciones de acuerdo con criterios éticos y metodológicos adecuados.

ABSTRACT Objective. To explore the association between reporting conflict of interest (COI) and having a positive outcome toward vaping in scientific articles. Methods. A cross-sectional study that analyzed a sample of 697 articles published between 2017 and 2020 regarding vaping. Information on the reporting of COI, type of COI (no conflict, conflict with the tobacco industry, pharmaceutical industry, or other), and country of publication were collected. To explore the association between reporting COI and having a positive result for vaping, two logistic regression models were fitted, both adjusted by country of publication. Results. From 88 articles that reported COI, 23 reported COI with the tobacco industry, 44 with the pharmaceutical industry, and 21 reported another type of conflict. We found that reporting any type of COI increased by 4.7 times the odds (OR 4.70; 95% CI [2.89, 7.65]) of having a positive result for vaping. Additionally, compared to other countries, manuscripts published in England had 2 times higher odds (OR 2.40; 95% CI [1.16, 4.98]) of reporting a positive result for vaping. Reporting COI with the tobacco and pharmaceutical industries increased the odds of favorable results by 29 times (OR 29.95; 95% CI [9.84, 90.98]) and 2 times (OR 2.87; 95% CI [1.45, 5.69]), respectively. Conclusions. In scientific articles, reporting COI and having positive results for vaping are highly associated. COI should be considered and caution should be exercised when using data for policy-making.


RESUMEN Objetivo. Explorar la relación entre notificar la existencia de conflictos de intereses y tener un resultado positivo con respecto al vapeo en los artículos científicos. Métodos. Se realizó un estudio transversal que analizó una muestra de 697 artículos sobre vapeo publicados entre 2017 y 2020. Se recopiló información sobre la notificación de la existencia de conflictos de intereses, los tipos de conflicto (sin conflicto o conflicto con la industria tabacalera, con la industria farmacéutica o con otras industrias) y el país de publicación. Para explorar la relación entre notificar la existencia de conflictos de intereses y tener un resultado positivo respecto del vapeo, se adaptaron dos modelos de regresión logística, y ambos se ajustaron por país de publicación. Resultados. De los 88 artículos en los que se notificó la existencia de conflictos de intereses, 23 informaron sobre conflictos de intereses con la industria tabacalera, 44 sobre conflictos con la industria farmacéutica y 21 sobre otros tipos de conflicto. Se determinó que la notificación de cualquier tipo de conflicto de intereses incrementó en 4,7 veces las probabilidades (OR 4,70; IC del 95 % [2,89; 7,65]) de obtener un resultado positivo con respecto al vapeo. Además, en comparación con otros países, los artículos publicados en Inglaterra mostraron probabilidades 2 veces más altas (OR 2,40; IC del 95 % [1,16; 4,98]) de comunicar un resultado positivo respecto del vapeo. Informar sobre la existencia de conflictos de intereses con las industrias tabacalera y farmacéutica multiplicó las probabilidades de resultados favorables por 29 (OR 29,95; IC del 95 % [9,84; 90,98]) y por 2 (OR 2,87; IC del 95% [1,45; 5,69]), respectivamente. Conclusiones. En los artículos científicos, existe una clara relación entre notificar la existencia de conflictos de intereses y tener un resultado positivo con respecto al vapeo. Se debe considerar el conflicto de intereses y actuar con precaución al emplear estos datos para la formulación de políticas.


RESUMO Objetivo. Examinar a associação entre declarar conflitos de interesses e obter resultados favoráveis sobre os cigarros eletrônicos em artigos científicos. Métodos. Estudo transversal que analisou uma amostra de 697 artigos sobre cigarros eletrônicos publicados entre 2017 e 2020. Foram coletadas informações sobre a declaração de conflito de interesses, tipo de conflito (ausência de conflito, conflito com a indústria tabagista ou farmacêutica, ou outras formas de conflito) e o país de publicação do estudo. Dois modelos de regressão logística foram criados, com ajuste por país de publicação, para examinar a associação entre declarar conflitos de interesses e obter resultados favoráveis para os cigarros eletrônicos. Resultados. De 88 artigos que continham a declaração de conflitos de interesses, 23 informaram conflitos com a indústria tabagista, 44 com a indústria farmacêutica e 21 outras formas de conflito. Declarar qualquer tipo de conflito de interesses foi associado a uma probabilidade 4,7 vezes maior (OR 4,70; IC 95% [2,89, 7,65]) de obter resultados favoráveis aos cigarros eletrônicos. Quanto aos países de publicação, verificou-se uma probabilidade duas vezes maior de os artigos publicados na Inglaterra (OR 2,40; IC 95% [1,16, 4,98]) informarem resultados favoráveis para os cigarros eletrônicos. Quando se declarou o conflito de interesses com as indústrias tabagista e farmacêutica, a chance de resultados favoráveis nos estudos foi 29 vezes maior (OR 29,95; IC 95% [9,84, 90,98]) e 2 vezes maior (OR 2,87; IC 95% [1,45, 5,69]), respectivamente. Conclusões. Observa-se uma forte associação entre declarar o conflito de interesses e obter resultados favoráveis para os cigarros eletrônicos em artigos científicos. O conflito de interesses deve ser levado em consideração, recomendando-se cautela ao utilizar os dados de estudos na formulação de políticas.

Along with the increasing global prevalence of myopia, severer myopia brings concerns about a higher risk of potentially blinding complications.Myopia is becoming one of the major causes of vision impairment and blindness in the world, and the myopia prevention and control has been a focus of the whole society.Management of myopia includes more outdoor activities, medications, orthokeratology, peripheral defocus soft contact lens, progressive addition lenses and bifocal lenses, etc.These methods have raised ethical disputes among regulatory bodies, manufacturers, academics and clinicians, who share an ethical responsibility to ensure that the products used for myopia control are safe and efficacious and that patients understand the benefits and potential risks of such products.The IMI- Industry Guidelines and Ethical Considerations for Myopia Control Report published in 2019 highlights the ethical challenges and provides stakeholders with recommendations and guidelines in decision-making, production development, spreading of research progress, prescribing, financial support and advertising of such treatments.Some common problems in combination with the latest clinical studies and relevant evidence-based guidelines were reviewed and reasonable solutions were recommended in this article.

El este tercer trabajo se presenta el análisis comparativo de las últimas pautas de CIOMS 2016, realizado por estudiantes de la VI Cohorte de la Maestría en Bioética como parte de la evaluación de la asignatura Bioética e Investigación. En estas pautas se abordan de forma novedosa temas tan importantes como la investigación en grupos vulnerables, en situaciones de desastre y brotes de enfermedades de orden natural o creadas por el hombre e investigaciones con conglomerados, así como la compensación por daños, manejo de datos cuando se utiliza el entorno virtual y herramientas digitales y el conflicto de interés, lo cual proporcionará a los investigadores un aporte en su formación y una rápida adaptación a la nueva propuesta CIOMS(AU)


In this third work, the comparative analysis of the latest CIOMS 2016 guidelines, carried out by students of the VI Cohort of the Master in Bioethics, as part of the evaluation of the subject Bioethics and Research is presented. These guidelines deal in a novel way with important issues such as research in vulnerable groups in situations of disaster and outbreaks of natural or man-made diseases and investigations with clusters, as well as compensation for damages, data management when the virtual environment and digital tools and the conflict of interest are used, which will provide researchers with a contribution in their training and a rapid adaptation to the new CIOMS proposal(AU)

Resumen Se reflexiona en torno al diseño normativo que regula la investigación en Colombia y las prácticas de los investigadores desde un enfoque ético, abordando asuntos pro blemáticos en las ciencias de la salud, las ciencias sociales y las disciplinas ambien tales, para concluir que se necesita fortalecer el control del Estado en estos temas, la actualización de la normatividad y una mayor formación de los investigadores y miembros de Comités de Ética en investigación, especialmente en: la protección de los participantes, el cuidado de los animales, los conflictos de interés, el manejo de comunidades y de grupos vulnerables, los protocolos comunitarios, el consentimien to informado, la consulta previa, el cuidado de la biodiversidad y el acceso a recursos genéticos.


Abstract This paper reflects on the normative design, which regulates research in Colombia and the practices of researchers from an ethical approach, by addressing critical issues in the health sciences, social sciences, and environmental disciplines, to conclude that it is necessary to strengthen state control in these issues, the updating of the regulations and greater training of researchers and members of Ethics Committees in research , especially in the protection of participants, animal care, conflicts of interest, the management of communities, and vulnerable groups, community pro tocols, informed consent, prior consultation, biodiversity care, and access to genetic resources.

ABSTRACT

Abstract Platelet-rich plasma is derived from centrifuging whole blood. There is increasing interest in the sports medicine and athlete community about providing endogenous growth factors directly to the injury site, using autologous blood products such as platelet-rich plasma. The aim of the present study is to evaluate the association between research financing, conflict of interests, level of evidence and author affiliation with the interpretation of results in articles published on platelet-rich plasma therapy in musculoskeletal ailments. A review of the current literature was performed. The outcome was classified as favorable or unfavorable. The declaration of conflict of interests and the type of funding was extracted from each article. The financing was classified as industry-sponsored; not industry-sponsored; or unidentifiable. The level of evidence was categorized from I to IV. Higher positive outcomes were observed in 134 studies with industry sponsorship compared with not industry-sponsored studies (odds ratio [OR]: 0.26; 95% confidence interval [95%CI]: 0.08-0.85; p < 0.05). Compared with level of evidence I, levels II and IV increase the probability of positive outcomes by 12.42 times (p < 0.01) and 10.97 times (p < 0.01) respectively. Proportionally, industry-sponsored studies are more likely to present positive results, as well as articles with a lower quality of evidence.


Resumo O plasma rico em plaquetas é derivado da centrifugação do sangue total. Há um interesse crescente, na medicina esportiva e na comunidade atlética, no fornecimento de fatores de crescimento endógeno diretamente ao sítio da lesão, usando componentes sanguíneos autólogos, como o plasma rico em plaquetas. O objetivo deste estudo é avaliar a associação entre financiamento de pesquisa, conflito de interesses, nível de evidência e afiliação dos autores com a interpretação dos resultados em publicações sobre terapia com plasma rico em plaquetas nas doenças osteomusculares. Foi realizada uma revisão da literatura atual. O desfecho foi classificado como favorável ou desfavorável. A declaração de conflito de interesses e o tipo de financiamento foram extraídos de cada artigo. O financiamento foi qualificado em patrocínio industrial; não patrocinado pela indústria; ou não identificável. O nível de evidência foi categorizado de I a IV. Foram obtidos os resultados positivos mais altos com 134 estudos financiados pelo setor industrial, em comparação com estudos não financiados pela indústria (razão de probabilidades [RP]: 0,26; intervalo de confiança de 95% [95%IC]: 0,08-0,85; p < 0,05). Em comparação com o nível de evidência I, os níveis II e IV aumentam a probabilidade de resultado positivo em 12,42 vezes (p < 0,01) e 10,97 vezes (p < 0,01), respectivamente. Demonstrou-se que, proporcionalmente, estudos patrocinados pela indústria têm maior probabilidade de apresentar resultados positivos, bem como artigos com menor qualidade de evidência.

Resumen: Para combatir la mala nutrición en todas sus formas es necesario integrar acciones desde la sociedad en su conjunto. El interés superior de los actores involucrados en generar estas acciones debe ser la salud. No obstante, existen actores con conflictos de interés que interfieren en el diseño, planeación, implementación y monitoreo de las políticas derivadas de dichas acciones. A fin de mitigar el efecto adverso que generan estos conflictos desde la etapa de generación de evidencia hasta el diseño e implementación de las políticas, se propone este código. Se invita a la comunidad de nutrición y salud a adoptarlo, promoverlo y sumarse al mismo, para favorecer el avance de las acciones y políticas necesarias sin interferencia de la industria y resolver el problema de la mala nutrición.


Abstract: To combat malnutrition in all its forms, actions must be integrated from society as a whole. The main interest of the actors involved in these actions should be health. However, there are actors with conflict of interest that interfere in the design, planning, implementation and monitoring of public health nutrition policies. In order to mitigate the adverse effect that result from these conflicts, from evidence generation to the design and implementation of policies, this Code is proposed. It invites the nutrition and health community to adopt it, promote it and subscribe it, and to favor the advancement of actions and policies without industry interference to address the problem of malnutrition.

Abstract Introduction: Physicians' main responsibility is to practice medicine for the benefit of patients. However, there are situations where secondary interests affect this commitment and give rise to conflicts of interest. Objective: To analyze currently available systematic reviews and meta-analyses on conflicts of interest in medicine to summarize relevant evidence in this regard. Material and methods: A literature search was performed in the MEDLINE and LILACS databases using the following search strategy: systematic reviews and meta-analysis on conflicts of interests in medicine published in serialized scientific journals; no publication time or language limits were applied. Studies that met the inclusion criteria were grouped according to the medical activity they assessed, and information on the type and number of studies and conclusions of all publications included in the review was collected. Results: 29 publications were included, and they were classified as follows: studies based on research articles, on clinical practice guidelines, on clinical practice, and on patient-oriented papers. Conclusions: It was found that the authors of the original research papers included in the meta analyses and systematic reviews analyzed here do not always state if they have conflicts of interest or not. Nevertheless, when said conflicts are reported, they tend to present results favoring the drugs or medical technologies of their sponsor.


Resumen Introducción. La principal responsabilidad de los médicos es la de actuar en beneficio de los pacientes; sin embargo, existen situaciones en las cuales surgen intereses secundarios que pueden afectar este compromiso y generar conflictos de intereses. Objetivo. Analizar las revisiones sistemáticas y los metaanálisis actualmente disponibles en la literatura sobre el conflicto de intereses en medicina para sintetizar la información al respecto. Materiales y métodos. Se realizó una búsqueda en las bases de datos MEDLINE y LILACS mediante la siguiente estrategia de búsqueda: revisiones sistemáticas y metaanálisis sobre conflictos de intereses en medicina publicados en revistas científicas seriadas; no se aplicaron restricciones de idioma o año de publicación. Los estudios que cumplieron con los criterios de inclusión fueron agrupados según la actividad médica evaluada; además, de cada uno de ellos se extrajo la cantidad y el tipo de estudios y las conclusiones. Resultados. Se seleccionaron 29 publicaciones que se agruparon en estudios basados en artículos de investigación, en guías de práctica clínica, en la práctica clínica, y en publicaciones orientadas a los pacientes. Conclusiones. Los estudios originales incluidos en las revisiones sistemáticas y los metaanálisis analizados en el presente estudio no siempre reportan los conflictos de intereses; sin embargo, cuando estos se mencionan, hay una tendencia a presentar resultados que favorecen el medicamento o la tecnología del patrocinador.

ABSTRACT

Objetivo: demonstrar que o processo de mediação de conflitos se apresenta como resposta adequada na prevenção, gestão e resolução de divergências, onde existam relações continuadas e o interesse em construir soluções efetivas, globais e duradoras. Metodologia: o presente trabalho é baseado em uma revisão bibliográfica e na experiência profissional da autora, por meio das suas intervenções e vivências práticas, com mais de 5.000h de experiência só em mediação. Resultados: na sua maioria os conflitos entre as diferentes subculturas na saúde ­ médica, enfermagem, administrativa, relações com paciente e família, bem como, mais recentemente, a tecnologia de informação e robótica na saúde ­ têm relação direta com distintas formas de trabalho, autoridade e legitimidade, compreendidos como divergentes e maioritariamente entendidos como incompatíveis e inconciliáveis. De igual forma, é indispensável conhecer e incorporar na organização processos de diálogo e negociação consensual, voluntários e confidenciais, como a mediação, facilitados por um terceiro imparcial, sem poder de decisão ou julgamento. Resulta confirmada a necessidade de as lideranças na saúde saberem lidar com a complexidade dos conflitos, que permanecem entre as diferentes subculturas. Organizações e gestores que utilizam processos consensuais de forma técnica potencializam experiências positivas, bem como melhoram indicadores de qualidade no ambiente social organizacional. Conclusão: espera-se que os gestores médicos, clínicos e hospitalares desenvolvam competências de prevenção, gestão e resolução consensual de conflitos, sendo necessário desenvolver e aprofundar o triângulo do conhecimento do processo de mediação.


Objective: to demonstrate that the conflict mediation process presents itself as an adequate response in the prevention, management and resolution of differences, where there are ongoing relationships and the interest in building effective, global and lasting solutions. Methodology: the present work is based on a bibliographic review and on the author's professional experience, through her interventions and practical experiences, with more than 5,000 hours of experience in mediation alone. Results: most of the conflicts between the different subcultures in health - medical, nursing, administrative, patient and family relations, as well as, more recently, information technology and robotics in health - are directly related to different forms of work, authority and legitimacy, understood as divergent and mostly understood as incompatible and irreconcilable. Likewise, it is essential to know and incorporate consensual, voluntary and confidential dialogue and negotiation processes in the organization, such as mediation, facilitated by an impartial third party, with no decision or judgment power. It is confirmed the need for health leaders to know how to deal with the complexity of conflicts, which remain between the different subcultures. Organizations and managers who use consensual processes in a technical way enhance positive experiences, as well as improve quality indicators in the organizational social environment. Conclusion: it is expected that medical, clinical and hospital managers develop competences for the prevention, management and consensual resolution of conflicts, being necessary to develop and deepen the knowledge of the mediation process


Objetivo: este artículo tiene como objetivo demostrar que el proceso de mediación de conflictos se presenta como una respuesta adecuada en la prevención, gestión y resolución de diferencias, donde existen relaciones continuas, así como un interés en construir soluciones efectivas, globales y duraderas. Metodología: el presente trabajo se basa en la revisión bibliográfica, así como en la experiencia profesional de la autora, a través de sus intervenciones y experiencias prácticas, con más de 5,000 horas de experiencia solo en mediación. Resultados: los conflictos entre las diferentes subculturas en salud (médica, enfermería, administrativa, en la relación con el paciente y la familia, así como, más recientemente, con la inclusión de la tecnología de la información y la robótica en la salud), la mayoría de ellas tienen una relación directa con diferentes formas: trabajo, autoridad y legitimidad, entendido como divergente, entendido principalmente como incompatible e irreconciliable. Asimismo, es esencial conocer e incorporar procesos de diálogo y negociación consensuales, voluntarios y confidenciales en la organización, como la mediación, facilitados por un tercero imparcial, sin decisión ni juicio. Se confirma que los líderes de salud necesitan saber cómo lidiar con la complejidad de los conflictos, que permanecen entre las diferentes subculturas. Las organizaciones y gerentes que usan procesos consensuados de manera técnica mejoran las experiencias positivas, así como también mejoran los indicadores de calidad en el entorno social organizacional. Conclusión: se espera que los gerentes médicos, clínicos y hospitalarios desarrollen competencias para la prevención, el manejo y la resolución consensuada de conflictos, y es necesario desarrollar y profundizar el triángulo de conocimiento del proceso de mediación.

A potential risk of conflict of interest currently exists in research.  The financial considerations play a greater role in the decisions that are necessary in the process of research. Traditional pharmaceutical companies began to invest, not just in the commercial development of biomedical discoveries for the marketplace, but in the research centres that could deliver the discoveries.  Cooperation and interdependence between industry and research institutions is not inherently wrong, but these unions must be regulated and managed through regulatory mechanisms. When a financial relationship is been disclosed, it would be closely evaluated in order to determine the risk of an undue influence leading to bias, or loss of scientific objectivity. To sum up, a conflict of interest is a potential, but not a certain occurrence. Banning funding for university research by industry is unrealistic.  The only effective way to proceed is to implement oversight and regulation that makes both industry funding entities and researchers aware that their activities will be monitored for the benefit of public safety as the more valued concern.

ABSTRACT The participation of the ultra-processed products industry in efforts to reduce obesity and diet-related non-communicable diseases has been questioned, especially because there is evidence of its interference in policy-making processes. This article builds on the Collective Action Theory and the literature of political science to discuss the role of this sector as a special interest group that uses its significant economic power to influence government decisions in its favor. In Brazil, its participation occurs mainly with industry associations. However, it has not yet been established whether their interests prevail in the decision-making process. It has been suggested that research should be carried out to determine the degree of success of their actions, identifying the conditions associated with the convergence of policy results with their interests and indicating to what extent civil society organizations are able to make public interests override private ones.


RESUMO A participação da indústria de produtos ultraprocessados nos esforços para reduzir a obesidade e doenças crônicas associadas à má alimentação tem sido questionada, sobretudo porque há evidências de sua interferência no processo de formulação de políticas públicas. Este artigo se alicerça na teoria da ação coletiva e na literatura de ciência política para discutir o papel desse setor como um grupo de interesse especial que utiliza seu significativo poder econômico para influenciar decisões governamentais em seu favor. No Brasil, sua atuação ocorre principalmente por meio de associações. Ainda não se pode assegurar, todavia, que seus interesses prevaleçam no processo decisório. Sugere-se a realização de pesquisas que determinem o grau de sucesso de suas ações, identifiquem as condições associadas à convergência dos resultados das políticas com seus interesses e apontem em que medida as organizações da sociedade civil são capazes de fazer os interesses públicos se sobreporem aos privados.

Objective: Identify and characterise the food industry's involvement in nutrition and dietetics national and regional events in Latin America and the Caribbean. Design: Between February and April 2020, we conducted desk-based searches for nutrition and dietetics events held in the region between January 2018 and December 2019. Online freely accessible, publicly available information was collected on the involvement of the food industry through: sponsorship of events; sponsorship of sessions; speakers from the food industry; scholarships, fellowship, grants, awards and other prizes and; exhibition space/booths. Setting: Nutrition and dietetics events in Latin America and the Caribbean. Results: Thirty-one events held in twenty countries of the region had information publicly available online at the period of data collection. There was a lack of transparency on the involvement of industry actors in these events. When information was publicly available, we found that a total of ninety-two food industry actors sponsored 88 % of these events. Conclusions: There is a mostly unreported, but likely extensive, involvement of food industry actors in nutrition and dietetics events in Latin America and the Caribbean.(AU)

ABSTRACT Objective: To describe simulated scenes on conflict management in nursing care, created by university students; and to identify the skills and attitudes needed by the nursing student to manage conflicts in health care. Methods: This is a qualitative study conducted with 28 university nursing students from a public college located in the city of Boa Vista, Roraima. The strategy adopted for the production of data was the simulation of scenes of conflict. The data were analyzed according to Bardin's methodology. Results: Four conflict scenes were produced, involving nurses and nursing technicians, health managers, multiprofessional health teams, and patient care. In this context, 274 record units were identified regarding skills and attitudes for nurses to manage conflicts. Final Considerations: The skills and attitudes essential for conflict management were: effective verbal communication, body language observation, knowing how to listen, negotiate, make decisions, be neutral, impartial, and how to lead democratically.


RESUMEN Objetivo: Describir escenas simuladas sobre gestión de conflictos en el cuidado de enfermería creadas por estudiantes universitarios; e identificar las competencias y actitudes necesarias al estudiante de Enfermería para administrar conflictos en el cuidado en salud. Métodos: Estudio cualitativo realizado con 28 estudiantes universitarios de Enfermería matriculados en una institución pública de enseñanza superiora localizada en el municipio de Boa Vista, Roraima. La estrategia adoptada para producción de los datos ha sido la simulación de escenas de conflictos. Los datos brutos han sido analizados según Bardin. Resultados: Se produjeron cuatro escenas de conflictos, envolviendo enfermero y técnicos de enfermería, gestor de salud, equipo multiprofesional de salud y el paciente cuidado. En total, han sido identificadas 274 unidades de registros que versan sobre competencias y actitudes para el enfermero administrar conflictos. Consideraciones finales: Han sido identificadas las competencias y actitudes indispensables para gestión conflictos: comunicación verbal efectiva, observación corporal, saber escuchar, negociar, tomar decisiones, ser neutro, imparcial y liderar democráticamente.

@#Objective To evaluate and analyze the panel composition and conflict of interest management of clinical practice guidelines of rehabilitation.Methods Clinical practice guidelines of rehabilitation were systematically retrieved from PubMed, EMBASE, China Biology Medicine disc, CNKI, Wanfang Data and Medlive database, the National Institute for Health and Care Excellence, National Guideline Clearinghouse, The Scottish Intercollegiate Guidelines Network, World Health Organization (WHO) and Guidelines International Network until January 11, 2020. Two researchers independently screened and cross-checked the literatures, and extracted the basic information of included guidelines, including the title, formulating institution, published journol, as well as the expert group, the number of expert group, the specific division of labor and their discipline, affiliation and the geographic location, the statement and management of conflict of interest, types of conflicts of interest, and whether to accept fund and the source of the fund.Results A total of 84 guidelines were finally included, 17 domestic ones and 67 foreign ones, in which, 52 (61.9%) reported the expert panel; 22 (26.2%) reported the number of expert panel groups, 21 (25.0%) indicated the specific division of work among the members of the expert panel, 74 (88.1%) reported the names of the members of the expert panel, 47 (56.0%) reported the subjects and specialties of the panelists, 70 (83.3%) reported the affiliation and location of panelists, 14 (16.7%) mentioned the management of conflicts of interest, and 25 (29.8%) reported if there were conflicts of interest. Only five of the 16 funded guidelines stated that there was no conflict of interest between the funding and the development of the guidelines. Among them, the reporting rate of expert panel was significantly higher in foreign countries than in China (χ2 = 9.542, P < 0.01), the reporting rate of name of expert panel members and specific division of labor were higher in foreign countries than in China ( χ2 > 4.155, P < 0.05), and the reporting rate of conflict of interest management was also higher in foreign countries than in China ( P < 0.05). There was no significant difference in whether there was a conflict of interest, whether it was funded and the type of funding at home and abroad ( P > 0.05). Conclusion In gerenal, clinical practice guidelines of rehabilitation published at home and abroad are necessary to be improved in the reporting quality of expert group formulation and division of labor, conflict of interest reporting and management. It is proposed that future guideline developers should follow the WHO Handbook for Guideline Development, assign roles of experts, strengthen the management and reporting of conflicts of interest, and standarderize the development process and reporting of the guidelines.
